Medication reconciliation post-discharge is a high-stakes, manual bottleneck where discrepancies between discharge orders, pre-admission regimens, and home inventory cause preventable readmissions. A custom multi-agent workflow automates this by deploying specialized agents for extraction, comparison, and verification. The architecture ingests discharge summaries from Epic or Cerner via FHIR, extracts medication lists using fine-tuned LLMs, and compares them against historical EHR data and pharmacy claims. Identified discrepancies are flagged with confidence scores and routed through an approval gate, automating a traditionally error-prone, labor-intensive process for health systems.
