This workflow automates that entire sequence. An orchestrator, built on frameworks like LangGraph, triggers upon appointment confirmation in the practice management system (e.g., Epic, Athenahealth). It fetches patient context and appointment type, then dynamically assembles a relevant questionnaire—pulling standardized forms for a cardiology consult versus a routine physical. The system pushes this via the patient portal API and initiates automated SMS/email reminders to drive completion.
