A postmortem (or incident postmortem) is a structured, blameless analysis and documentation process conducted after a significant system incident, outage, or failure. Its primary goal is to establish a root cause analysis, document the timeline of events, assess the business impact, and, most critically, produce a list of actionable remediation items to prevent recurrence. In the context of multi-agent system orchestration, a postmortem would analyze failures in agent coordination, communication protocols, or workflow logic.
